This piece is internally threaded, this means the screw threads are attached to the ball.  The small screw threads into the end of a post. (both ends unscrew)

This allows for a smooth surface on the jewellery being inserted into the piercing, which prevents microscopic tears to your skin.  The ends seal perfectly into the bar, so there are no gaps for buildup of debris.
